1.     Complete and sign the Homeowner Assistance Application Form.

2.     Make a copy of all documents showing proof of income.

3.     Mail all above items in one envelope to:

After we have received this information we will contact you. If you are eligible, we will call you to schedule a time to preview and assess your home's needs.  If we are able to meet your needs, the work will be done by volunteer teens and adults during scheduled work sessions.  Please be patient with us.  Our current backlog is one to two years.
